One of the main physical manifestations of our progress at the local level is our HealthHUBs. Research shows that patients with an ongoing medical condition struggle to get the coordinated care and health services they need at the lowest cost site of care. HealthHUBs offers more clinical services and features a broader range of health care offerings to help people better manage chronic conditions. And while the HealthHUBs may be the most tangible demonstration of our goal to transform care, our work to improve patient care goes much deeper. We have more than 35 years of experience delivering nutrition and infusion therapies, education and other services for patients at critical points in their journey, such as discharging from the hospital and transitioning home. We currently serve over 50,000 patients each month in home-based and ambulatory clinic settings, playing an important role with hospital systems, reducing readmissions and managing the requirements of value-based reimbursement models.


Our research shows that home infusion care is a promising model that is both cost and clinically effective and is overwhelmingly preferred by patients when intravenous therapy is required. We have a significantly lower all-cause unplanned 30-day readmission rate than the national average and have seen up to a 24 percent reduction in readmissions. We also are now offering more personalized services into people's homes and communities when it's convenient for them - and on their terms.
